SCAN Health Plan is a not-for-profit Medicare Advantage health plan that provides health coverage and care coordination services to seniors. It serves several hundred thousand older adults, primarily in California, Arizona, Nevada, and Texas. Founded in 1977 by a group of senior activists as the Senior Care Action Network, SCAN is headquartered in Long Beach, California and is one of the largest not-for-profit Medicare Advantage plans in the country.
